Your report must be submitted in Arial 10 pt and double spaced. It should comprise the following:

Cover sheet (Word document), with the following details:

Name, job title, address, telephone number and e‑mail address
The category you are entering, e.g. Secondary Care Award
Title of report
Level of your involvement in this project/report
Technician registration number and qualifications taken
Name and job title of your Manager (Please can all Managers send an email to esther.joberns@aah.co.uk confirming their approval and awareness of the entry being submitted)
Where you heard about the award
Please confirm that the report is original and has not been submitted for publication elsewhere
Word count of report (Please note that the maximum word count is 3000)

Abstract – (maximum 300 words)
Introduction, including aims
Method (materials and participants involved, how measurements were taken and how many, what calculations were used)
Results e.g. in tables or graphical format
Discussion
Personal input, i.e. please indicate which areas of the report you have carried out on your own
Appendices/supporting material
